Laser refractive surgery of the cornea is at the current state widely accepted as a safe and effective standard procedure for surgical correction of low to moderate ametropia (i.e., myopia/hyperopia/astigmatism). Current literature and own data reveal the high safety and efficacy of the procedures. According to Price et al. [2], the risk of laser refractive surgery is comparable to contact lens wear regarding safety and side effects. However, complications still occur and can subsequently result in reduced efficacy and safety of the procedure. Both human and technical sources of error exist. The first step in the string of pearls of complication management is the precise detection and correct diagnosis of a complication followed by its targeted immediate management. In light of millions of refractive laser surgeries performed worldwide per year, we aim to provide a comprehensive textbook on complications and their management in refractive and therapeutic laser surgery of the cornea. The list of possible complications includes intraoperative flap cutting errors, decentration of optical zone, incorrect transfer of laser parameters, and postoperative complications. Infectious keratitis and keratectasia are among the most severe postoperative complications and can result in permanent loss of vision. Although the focus of this textbook is complications in corneal laser surgery, we include a chapter about corneal topography/tomography. Knowing and differentiating between regular and irregular shape of the cornea is mandatory for a reliable, safe, and straightforward screening process of refractive surgery candidates. Establishing a consensus for laser refractive surgery indications/contraindications and treatment based on the current scientific knowledge can significantly improve surgical outcome and patient satisfaction.
